What Should You Talk About When You Have Nothing to Say in Therapy?

Sometimes a person arrives at therapy knowing something feels wrong but cannot identify what to discuss. The silence can feel awkward, especially during early sessions. What can someone talk about when they do not know where to start, and is it normal for therapy sessions to feel like this?
 
When I have nothing to say in therapy, I’d probably start with that exact feeling. Saying, “I don’t know what to talk about today” can open things up naturally. Sometimes the silence itself says something, especially if I’m avoiding a topic without realizing it.
